Its a Tricky question about evidencing time frame one of my friend has received his visa with in 4 days MASHALLAH.
In my case it took 7 days but after wards they took like 10 days, 15 days and in one case more than a month. but one thing is sure if you have mentioned VISA SECTION on your envelope then you will receive your passports with visa soon very soon INSHALLAH.
No need for taking any other permission my friend .. Its simple just buy one ticket and fly out ;-)

dear as per law you should get the protector as you have 457 ENS (working visa)

now you have two options either go to airport with 10K in pocket (one window benefit) or get to three diff offices and spend 5K.

but in my opinion getting stuck at airport should be your last choice.

dear protector is required (as per law) for only "work visa" holders, those leaving PAkistan on immigration PR visa it does not valid or required nor it will be asked for.

for your second question NICOP is your identification "made in english" for your help to prove that you are Pakistani, but if you have valid passport which is also made in english can help you to prove yourself, however please visit the below link for further reading.

I agree with you that there is no discrimination in DIAC procedures for PAKISTANI Applicants.
Only difference is Pakistani applicants needs to go through some Additional SECURITY CHECKS which are some times called EXTERNAL CHECKS or INTEGRITY CHECKS.
These Security checks vary from case to case but at least time frame is 12 Months. If you can see the estimated time lines given by DIAC this is clearly mentioned that Time frame for HR countries is 24 Months for 176 Sub class.
Just for example my External checks consumed 12 Months after CO allocation. and you can see my time lines in my signature.

I have a question regarding Medicals. Can someone tell me how much does it cost in Karachi and whats the procedure plus how much time will it take.

I am thinking about front-loading the medical, is it a good idea ?
